چکیده

Aggressive neoplasms of the scalp need a wide and deep excision to obtain safe margins and tumor eradication [1]. These considerations sometimes lead to the inclusion of the periosteum as part of wide local excision. The consequence of large surgical excision of the scalp is the exposure of the skull bone with its poor vascularization, therefore a slow granulation and, eventually, a difficult healing by secondary intention. The exposed bone can be covered with local flaps but most often, this option, especially for large flaps, is not without complications (necrosis of the flaps, dehiscence, necessity of general anesthesia, etc.), hence free tissue transfer or use of dermal substitute are the only potential alternatives. Both these techniques have got the advantage to allow rapid coverage of larger surgical defects. On the other hand, they require a well vascularized recipient bed. Consequently, in such cases the outer table of the skull needs to be burred till bleeding is produced [2]. This procedure obviously complicates the reconstructive time, needing a general anesthesia, the presence of a neurosurgeon and increasing the patients’ morbidity. We successfully used a dermal substitute (acellular bovine tissue composed of native structurally intact collagen fibrils and elastin Matriderm® produced by Dr. Suwelack Skin & Health Care AG, Billerbeck, Germany) directly on the skull for reconstruction of the scalp following the excision of a sarcoma.

Repair of a Scalp Defect with Punctate Skull Fenestration and Skin Grafting

Reconstructing scalp defects can be challenging for a plastic surgeon. For this purpose, different procedures such as primary repair and coverage with skin grafts or flaps can be considered. Sometimes, the external table of the skull can be removed completely to the diploic space. At the same time or after the formation of granulation tissue, the diploic space can be covered with split-thicknes...

Single-Stage Full-Thickness Scalp Reconstruction Using Acellular Dermal Matrix and Skin Graft

Full-thickness scalp defects with exposed calvarium pose a reconstructive challenge, particularly in patients with extensive comorbidities. A single-stage acellular dermal matrix and split-thickness skin graft reconstruction represents a simple and valuable surgical approach to achieving durable scalp coverage without requiring a donor-site or prolonged treatment. Reconstruction of full-thickne...

One-Stage Reconstruction of Scalp after Full-Thickness Oncologic Defects Using a Dermal Regeneration Template (Integra)

The use of Dermal Regeneration Template (DRT) can be a valid alternative for scalp reconstruction, especially in elderly patients where a rapid procedure with an acceptable aesthetic and reliable functional outcome is required. We reviewed the surgical outcome of 20 patients, 14 (70%) males and 6 (30%) females, who underwent application of DRT for scalp reconstruction for small defects (group A...

Reconstruction of Chronic Complicated Scalp and Dural Defects Using Acellular Human Dermis and Latissimus Dorsi Myocutaneous Free flap

We present reconstruction of a complicated scalp-dura defect using acellular human dermis and latissimus dorsi myocutaneous free flap. A 62-year-old female had previously undergone decompressive craniectomy for intracranial hemorrhage. The cranial bone flap was cryopreserved and restored to the original location subsequently, but necessitated removal for a methicillin-resistant Staphylococcal i...

INTRODUCTION Surgical defect left following excision of a large and neglected scalp melanoma of can pose a significant challenge for the surgeon.
